New Patients

We welcome all new patients. As a new patient, you may complete and print your patient information form from this website. Please fill it out and bring it with you to your first visit. The information is necessary so that we may provide you with the best dental care possible. Generally, your first appointment will be for an examination only. Please try to obtain any x-rays from your dentist prior to your appointment. These will assist the doctor in determining your diagnosis. If necessary, we will take supplemental x-rays. If you cannot get your x-rays from your dentist, don’t worry, we can do the examination and get them at a later date. In the case that your needs are more of an emergency, please let us know and we will make every effort to see you as quickly as possible.
